dc.description.abstractBACKGROUND. Although the efficacy of mammography has been accepted as screening for breast cancer in developed countries, depending on the costs and gains in mortality, the results observed in Colombia and other Latin American countries, and in general in low-income countries, are not equally conclusive. The authors estimated the cost-effectiveness of mammography as a screening method for the early detection of breast cancer in women affiliated with a Promoting Health Entity (EPS) of Antioquia (Colombia). METHODS. For this study, a Markov state transition model was developed to simulate, given the result of mammographic screening, the diagnosis of breast cancer, the five-year survival and the cost of treatment in the women who received this test for the early detection of breast cancer. The benefit of mammography was modeled by assuming a stage change, in which cancers in women examined were more likely to be diagnosed at an earlier stage of the disease. The authors calculated the costs of this screening strategy and the potential impact on mortality, given the results of the model. RESULTS. The evaluation resulted in a total of 742 Life Years Saved (LYS) at a cost per LYS of COP $ 20,233,400.43 (USD $ 6,996.55), which represents 1.1 times the Gross Domestic Product Per Capita (PIB-PC) of the same year, so for this study screening mammography is considered a "Cost-effective" strategy. CONCLUSIONS The results obtained suggest that mammography for Antioquian women between 50 and 69 years old is currently a cost-effective strategy for the EPS to which they are affiliated, in the early detection of breast cancer, based on the threshold established by WHOspa
dc.description.abstractANTECEDENTES. Aunque se ha aceptado la eficacia de la mamografía como tamizaje para el cáncer de mama en los países desarrollados, en función de los costos y ganancias en mortalidad, los resultados observados en Colombia y otros países latinoamericanos, y en general en los países de bajos ingresos, no son igual de concluyentes. Las autoras estimaron la costo-efectividad de la mamografía como método de tamizaje para la detección precoz de cáncer de mama en mujeres afiliadas a una Entidad Promotora de Salud (EPS) de Antioquia (Colombia). MÉTODOS. Para este estudio, se desarrolló un modelo de transición de estado de Markov para simular, dado el resultado del cribado mamográfico, el diagnóstico de cáncer de mama, la sobrevida a cinco años y el costo del tratamiento en las mujeres que recibieron esta prueba para la detección temprana del cáncer de mama. El beneficio de la mamografía se modeló suponiendo un cambio de etapa, en el cual los cánceres en mujeres examinadas tenían más probabilidades de ser diagnosticados en una etapa más temprana de la enfermedad. Las autoras calcularon los costos de esta estrategia de detección y el impacto potencial en la mortalidad, dado los resultados del modelo. RESULTADOS. La evaluación dio como resultado un total de 742 Años de Vida Salvados (LYS) a un costo por LYS de COP $20.233.400,43 (USD $6.996,55), que representa 1.1 veces el Producto Interno Bruto Per Cápita (PIB-PC) del mismo año, por lo que para este estudio la mamografía de tamizaje se considera una estrategia “Costo-efectiva”. CONCLUSIONES. Los resultados obtenidos sugieren que la mamografía para las mujeres antioqueñas entre 50 y 69 años actualmente es una estrategia costo- efectiva para la EPS a la que están afiliadas, en la detección precoz del cáncer de mama, con base en el umbral establecido por la OMS.spa
