The cost-effectiveness of AFAED in treating PTNB in the NICU is unknown. Of interest are the possible economic implications of using it as standard treatment. This study evaluated the costs and effectiveness of this care. Methods. A CEA of AFAED vs. traditional care was performed in a cohort of PTNBW infants treated in a NICU in Barranquilla (Colombia). Direct costs of care were estimated from a third-party payer perspective. Weight gain and hospital stay were the measures of effectiveness. CER for each alternative and ICER were estimated. Results. The average cost of stay with AFAED was COP $28,642,524 (SD: 21,515,021) and with traditional treatment, COP $41,532,597 (SD: 35,649,081). The average length of stay for AFAED was 20.55 days (SD: 13.31) versus 29.00 (SD: 22.73). The average weight gain was higher in those treated with AFAED (29.66 g - SD: 66.38 - vs. 23.06 - SD: 95.12). The RCEI was COP$ -93,585.60 per g-day of weight gained. Discussion. The effectiveness indicator (average weight gain) was higher, and the average costs per patient were slightly lower in patients with AFAED, ment a better cost-effectiveness ratio of this treatment strategy. Simulation of different scenarios in which the cost of professional fees and length of stay in the NICU increased or decreased by 15% and 30% showed that AFAED is the dominant alternative

Importance The Global Burden of Diseases, Injuries, and Risk Factors Study 2019 (GBD 2019) provided systematic estimates of incidence, morbidity, and mortality to inform local and international efforts toward reducing cancer burden. Objective To estimate cancer burden and trends globally for 204 countries and territories and by Sociodemographic Index (SDI) quintiles from 2010 to 2019. Evidence Review The GBD 2019 estimation methods were used to describe cancer incidence, mortality, years lived with disability, years of life lost, and disability-adjusted life years (DALYs) in 2019 and over the past decade. Estimates are also provided by quintiles of the SDI, a composite measure of educational attainment, income per capita, and total fertility rate for those younger than 25 years. Estimates include 95% uncertainty intervals (UIs). Findings In 2019, there were an estimated 23.6 million (95% UI, 22.2-24.9 million) new cancer cases (17.2 million when excluding nonmelanoma skin cancer) and 10.0 million (95% UI, 9.36-10.6 million) cancer deaths globally, with an estimated 250 million (235-264 million) DALYs due to cancer. Since 2010, these represented a 26.3% (95% UI, 20.3%-32.3%) increase in new cases, a 20.9% (95% UI, 14.2%-27.6%) increase in deaths, and a 16.0% (95% UI, 9.3%-22.8%) increase in DALYs. Among 22 groups of diseases and injuries in the GBD 2019 study, cancer was second only to cardiovascular diseases for the number of deaths, years of life lost, and DALYs globally in 2019. Cancer burden differed across SDI quintiles. The proportion of years lived with disability that contributed to DALYs increased with SDI, ranging from 1.4% (1.1%-1.8%) in the low SDI quintile to 5.7% (4.2%-7.1%) in the high SDI quintile. While the high SDI quintile had the highest number of new cases in 2019, the middle SDI quintile had the highest number of cancer deaths and DALYs. From 2010 to 2019, the largest percentage increase in the numbers of cases and deaths occurred in the low and low-middle SDI quintiles. Conclusions and Relevance The results of this systematic analysis suggest that the global burden of cancer is substantial and growing, with burden differing by SDI. These results provide comprehensive and comparable estimates that can potentially inform efforts toward equitable cancer control around the world.

It was analyzed the percentage distribution of cases, as well as age-adjusted rates and burden indicators: It was analyzed the percentage distribution of cases, as well as age-adjusted rates and burden indicators: Years of Life Lost due to Premature Death (AVPP), Years of Life with Disabilty (AVD) and Life Lost Years due to disability (AVISA). Results: In 2019, 8.3% of patients received corresponded to arterial hypertension. The estimated prevalence in patients over 20 years old was 12.7%. From 2012 to 2019, 876 members died due to hypertension, with age-adjusted rates per-100,000, ranging from 10.42 to 15.96. The patients of AVPP were 8791.82, AVD: 13298.49 and AVISA: 25865.24. Differences were found to each indicator by department. Conclusion: A high burden of hypertensive diseases was found in affiliates of CAJACOPI EPS; the results are comparable with official data from the ASIS. The methodological limitations are inherent to the type of study, but the results obtained are considered an important input for decision making.

Objectives Our study compares two national COVID-19 vaccination plan strategies—high-risk prioritization and no prioritization—and estimates their cost-effectiveness compared with no vaccination, to generate possible recommendations for future vaccination plans. Methods We developed a Markov discrete-time, compartmental, deterministic model stratified by Colombian departments, healthcare workers, comorbidities, and age groups and calibrated to seroprevalence, cases, and deaths. The model simulates three scenarios: no vaccination, no prioritization of vaccination, and prioritization of high-risk population. The study presents the perspective of the health system of Colombia, including the direct health costs financed by the government and the direct health outcomes related to the infection. We measured symptomatic cases, deaths, and costs for each of the three scenarios from the start of the vaccination rollout to February 20, 2023. Results Both for the base-case and across multiple sensitivity analyses, the high-risk prioritization proves to be the most cost-effective of the considered strategies. An increment of US$255 million results in an incremental cost-effectiveness ratio of US$3339 per disability-adjusted life-year avoided. The simulations show that prioritization of high-risk population reduces symptomatic cases by 3.4% and deaths by 20.1% compared with no vaccination. The no-prioritization strategy is still cost-effective, with an incremental cost-effectiveness ratio of US$5223.66, but the sensitivity analysis the show potential risks of losing cost-effectiveness under the cost-effectiveness threshold (one gross domestic product per averted disability-adjusted life-year). Conclusions The high-risk prioritization strategy is consistently more cost-effective than the no-prioritization strategy across multiple scenarios. High-risk prioritization is the recommended strategy in low-resource settings to reduce the burden of disease.

Cost-effectiveness of COVID-19 vaccination in Colombia: a micro-simulation modelling analysis

To model the dynamics of the COVID-19 pandemic including new VOCs and vaccines and estimate the cost-effectiveness of the vaccination program in Colombia. We developed a SEIARHUDSq micro-simulation model through nine exhaustive and mutually exclusive states: susceptible, exposed, symptomatic-infected, asymptomatic-infected, recovered, hospitalized general ward, Intensive Care Unit, death, and sequela. SEIARHUDSq was run with data reported up to October 21, 2021, and adjusted as a system of differential equations based on epidemiological (cases, deaths, VOCs, vaccine effectiveness, etc.) and economic (vaccines prices and administration, direct medical costs of states, etc.) inputs obtained from national sources and systematic literature reviews. DALYs and ICERs showed the burden of COVID-19 and the cost-effectiveness of the vaccination program. Two scenarios, in addition to do-nothing, were modelled for infection rates (Beta) estimated by an agnostic model: 1) Beta=0.4 and 2) Beta=0.5, for 70 and 90% of vaccination coverages. The effect of the vaccines (Pfizer, Moderna, Sinovac, AstraZeneca, and Janssen) was assumed 14 after their application. Willingness-to-pay was set as one GDP per capita (US$5,376). We reported costs in American dollars (1$US=COP$3,702). In Do-nothing scenario the DALYs ranges between 2.3 and 2.4 million. By vaccinating the population, the burden of disease decreased to under 2.15 million DALYs. If 70% of the population were vaccinated, direct medical and vaccination costs (including administration) would be between US$2.1-2.4 billion for a Beta of 0.4-0.5, respectively (net costs). Costs would vary between US$2.3 and 2.4 billion at 90% simulated coverage. COVID-19 vaccination in Colombia have been cost-effective for Beta=0.4 scenario (ICER 70% coverage: US$995.5; ICER 90% coverage: US$1,837) and for Beta=0.5 (ICER 90% coverage: US$1,792). The scenario for Beta=0,5 (ICER 70% coverage) was dominated. COVID-19 vaccination in Colombia, prioritizing elderly and population with comorbidities, has been highly cost-effective in most of the modeled scenarios.

Objetivos. Establecer los determinantes económicos y sociales que se asocian con la situación nutricional de la primera infancia en el municipio de Manatí, Atlántico, en 2018. Población. Niños de 0 a 5 años del Municipio de Manatí, Atlántico. Antecedentes. La situación nutricional de una población refleja en gran medida su nivel de bienestar y puede ser utilizada para identificar inequidades que podrían afectar su productividad. La desnutrición crónica, identificada por el retraso en el crecimiento lineal o la talla baja de los niños, está asociada con menor desempeño escolar, así como con menor productividad y menor ingreso laboral en la vida adulta. El gran desafío en Colombia es disminuir las brechas de la desigualdad e inequidad para cumplir con los Objetivos de Desarrollo del Milenio en relación con disminuir la pobreza y la desnutrición materno-infantil. Al mismo tiempo también se observa en nuestro país, al igual que en otros países de América Latina, un incremento de la prevalencia de obesidad infantil, esta situación puede conllevar en la mayoría de los casos a obesidad en el adulto y aumento de los costos en la atención de salud por enfermedades crónicas no transmisibles. Según datos de la Encuesta Nacional de salud nutricional 2010, el departamento del Atlántico tiene cifras de bajo peso al nacer, delgadez y retraso en la talla superior a la media nacional. Métodos. Se realizó un estudio transversal, descriptivo en una muestra de niños de 0 a 5 años del Municipio de Manatí, Atlántico. Se analizarán variables sociodemográficas de los padres, la situación de la vivienda y se relacionarán con la situación nutricional de los niños. Resultados. Solo el 3,2% de los niños y el 2,1% de las niñas tienen una situación nutricional óptima. El 1,1% de los niños presentaron anemia. El 21,4% de los niños y de las niñas sufren algún grado de desnutrición. El 30,9% de los niños y el 30,4% de las niñas tienen algún grado de riesgo de padecer desnutrición. Se observó, además, que la mayoría de los servicios públicos están instalados en más del 90% de las viviendas, excepto el alcantarillado (63,2%). Se observaron desigualdades en la nutrición en contra del acceso a los servicios públicos de alcantarillado y aseo, vivir en zona rural, la práctica de la lactancia materna y el género femenino. Discusión. Los hallazgos del presente estudio corroboran las relaciones entre el estado nutricional y factores socioeconómicos como el género, el acceso a servicios públicos domiciliarios básicos, la vivienda en el área rural y la lactancia materna, encontrados por otros estudios y revisiones en el mundo, el continente y el país. Conclusión. La relación entre el estado nutricional de menores de cinco años y factores socioeconómicos de los hogares de los que provienen confirma la necesidad de atacar este tipo de problemática de salud pública desde el enfoque de los determinantes sociales de la salud.

Effectiveness of CoronaVac and BNT162b2 COVID-19 mass vaccination in Colombia: a population-based cohort study

Background In February 2021, Colombia began mass vaccination against COVID-19 using mainly BNT162b2 and CoronaVac vaccines. We aimed to estimate vaccine effectiveness (VE) to prevent COVID-19 symptomatic cases, hospitalization, critical care admission, and deaths in a cohort of 796,072 insured subjects older than 40 years in northern Colombia, a setting with a high SARS-CoV-2 transmission. Methods We identified individuals vaccinated between March 1st of 2021 and August 15th of 2021. We included symptomatic cases, hospitalizations, critical care admissions, and deaths in patients with confirmed COVID-19 as main outcomes. We calculated VE for each outcome from the hazard ratio in Cox proportionally hazards regressions (adjusted by age, sex, place of residence, diabetes, human immunodeficiency virus, cancer, hypertension, tuberculosis, neurological diseases, and chronic renal disease), with 95% confidence intervals (CI). Findings A total of 719,735 insured participants of 40 and more years were followed. We found 21,545 laboratoryconfirmed symptomatic COVID-19 among unvaccinated population, along with 2874 hospitalizations, 1061 critical care admissions, and 1329 deaths, for a rate of 207.2 per million person-days, 27.1 per million person-days, 10.0 per million person-days, and 12.5 per million person-days, respectively. We found CoronaVac was not effective for any outcome in subjects above 80 years old; but for people 40-79 years of age, we found two doses of CoronaVac reduced hospitalization (33.1%; 95% CI, 14.5−47.7), critical care admission (47.2%; 95% CI, 18.5−65.8), and death (55.7%; 95% CI, 32.5−70.0). We found BNT162b2 was effective for all outcomes in the entire population of subjects above 40 years of age, significantly declining for subjects ≥80 years. Interpretation Two doses of either CoronaVac in population between 40 and 79 years of age, or BNT162b2 among vaccinated above 40 years old significantly reduced deaths of confirmed COVID-19 in a cohort of individuals from Colombia. Vaccine effectiveness for CoronaVac and BNT162b2 declined with increasing age. Objectives: To estimate the prevalence and risk factors of post-COVID-19 condition after at least three months of hospital discharge in a retrospective cohort of patients from a health insurance company (Mutual SER EPS) living in the north of Bolivar, a department of Colombia. Methods: Clinical and sociodemographic data were collected from 181 COVID-19 patients who were hospitalized in the ward or intensive care unit (ICU) between March-2021 to August-2021. Patients who agreed to participate were evaluated by a structured telephone interview to identify persistent or new symptoms after at least three months of hospital discharge. Associations between at least one symptom and clinical and sociodemographic characteristics at admission were assessed through bivariate and multivariate logistic regression using a stepwise Akaike information criterion (AIC) selection. Adjusted odds ratios (aOR) are reported. Results: Mean age of patients was 56 years (SD: 17 years), 50.8% (n=92) were women and 23.2% (n=42) were previously admitted to ICU. One-hundred-three patients (56.9%) had at least one comorbidity; being hypertension (36.5%), type 2 diabetes mellitus (18.2%) and heart disease (8.28%) the most common. The presence of at least one symptom after three months of hospital discharge was reported by 141 (77.9%) patients. Most frequently symptoms were fatigue (54.1%), joint pain (45.3%), dyspnea (43.1%), sleep disorders (36.5%), chest pain (30.4%), and anorexia (30.4%). Symptoms were significantly more frequent in women (aOR: 2.45, 95% CI 1.14- 5.29, p=0.02) and ICU admitted patients (aOR: 3.10, 95% CI 1.02-9.45 p=0.05), but were not associated with age, having comorbidities, or a previous long hospital stay. Today, polypharmacy represents one of the major concerns of health systems in the world due to the risks associated with the inappropriate use of drugs and its impact on growing pharmaceutical spending. The aim of this study is to estimate the variation in the prevalence of polypharmacy during 2019-2021. Cross sectional descriptive study of polymedicated patients during 2019-2021 was developed. Data was extracted from the individual outpatient dispense records from a health insurance company in Colombia. Polypharmacy was defined as mild, moderate and severe when patients consumed among 5-9, 10-15 and ≥15 medications in at least a continuous month throughout the years 2019-2021. Prevalence and characteristics as age and gender in polymedicated patients were estimated. 429.967 patients were identified as consumers of medicines in 2021. The overall prevalence of polymedication was 29.2%, 3 pp. higher than 2019. The mean number of medications per patient was 6.79± 2.1 in 2019 and 6.93± 0.04 in 2021. Mild polymedication was about 26.9%, moderate 2.1% and severe 0.2% in 2021, 1.6pp., 0.9pp. and 0.1pp. higher than 2019 respectively. The average age of a patient in 2021 with mild, moderate and severe polymedication was 56.15 (CI95%: 56.1-56.2), 64.5 (CI 95%: The observed increase in polypharmacy could be influenced by the increase in the prescription rate and by a less and less interaction between the different prescribers. These, represent significant risks to the health of patients and can significantly impact pharmaceutical spending. This study contributes to decision-making on health risk management and drug consumption control, especially in the elderly population.

How expensive are hospitalizations by Covid-19? evidence from Colombia

Objectives This study aimed to estimate the direct medical costs due to hospitalizations by COVID-19 in Colombia and to identify their cost drivers in Colombia. Methods This is a retrospective cost-of-illness study of COVID-19 in Colombia. We estimated direct medical costs using data from patients insured to a Benefit Plan Administrator Company, between March 15, 2020 and May 29, 2020. Absolute and relative frequencies, averages, medians, and interquartile ranges (IQRs) were used to characterize the population and estimate the costs of hospitalized patients with COVID-19. We stratified the cost analysis by sex, age groups, comorbidities, and type of hospitalization (general ward and intensive care unit [ICU]). Cost drivers were calculated from a generalized linear model. Results We studied 113 confirmed patients, 51.3% men. On average, the hospital length of stay was 7.3 (± 6.2) days. A person hospitalized with COVID-19 reported median costs of $1688 (IQR 788-2523). In women, this cost was $1328 (IQR 463-2098); in men, this was 1.4 times greater. The median cost for ICU was $4118 (IQR 2069-5455), 3 times higher than those hospitalized only in the general ward. Admission to the ICU, having 1 comorbidity, length of stay, high blood pressure, having 5 comorbidities, and being treated in the city of Cartagena were statistically significant with direct medical costs. Conclusions Our study provides an idea of the magnitude of costs needed to hospitalize a COVID-19 case in Colombia. Other studies in Colombia have assessed the costs of hospitalization for infectious diseases such as influenza, costs significantly lower than those described here.

Impact of mobility restrictions on the dynamics of transmission of COVID-19 in Colombian cities

Our aim was to study the association between case rates and reductions in urban mobility in state capitals of Colombia. We designed an ecological time-series study to correlate the Colombian incidence rate with reductions in mobility trends of retail stores. The meta-analysis of β coefficients describing the association between case rates and reductions in mobility trends of retail stores resulted in a mean estimate of 0.0637 (95% confidence interval 0.027 to 0.101; p<0.001) with nearly 100% heterogeneity. We recommend continuing to consider mobility restrictions when the number of cases starts to climb in each local jurisdiction.

Indirect costs of diabetes mellitus in Colombia, 1998-2019

To estimate the indirect costs related to productivity losses attributable to premature mortality by diabetes mellitus (DM) in Colombia, 1998-2017. Retrospective analysis from the human capital approach. We estimated the potential productivity years of life lost (PYLLs) related to DM deaths. We obtained mortality data from death certificates of the Colombian statistics bureau. PYLLs were assessed considering the productive lifetime in Colombia (ranged from 18 to 57 years in women and up to 62 for men) and the age of death (adjusted for mid-period). Two scenarios were built to value premature mortality: 1) Lower loss: PYLLs were quantified. Between 1998-2019, Colombia reported 157,156 DM deaths, 81% occurred over 60 years of age, and 57% in women. Premature DM deaths accounted for 260,879 PYLLs, 62.4% in men and 57.6% in population aged 40-64 years. The non-discounted costs related to DM premature mortality range between US$942 million–1.7 billion throughout the study period. The economic burden associated with this disease in the discounted scenario ranges from US$573 million–1 billion. For 2019, these costs range between US$. We valued the economic burden due to DM premature deaths in Colombia. To complete this burden, it is necessary to include direct costs, out-of-pocket health expenditures, and indirect costs associated with loss of productivity owing to disability.

Infant immunization coverage in a colombian poor municipality in the post-conflict era

Colombia has an expanded program of immunization of >22 biologics mainly for younger than 6 years old. We aimed to estimate vaccination coverage in children under 6 years old in San Jacinto (Bolivar), in a post-conflict and COVID-19 pandemic scenario. We conducted a cross-sectional survey from October to November of 2021, in a Caribbean municipality of Colombia (24,910 population) mostly affected by the armed conflict. A two-stage cluster sampling was used with a precision of 5.5% and a design effect of 2, considering a non-response rate of 20%. We collected data on immunization record cards and other basic sociodemographic information from surveys of caregivers of children. Absolute and timely-vaccine coverages adjusted by age were. Out of 187 surveyed children, 159 (85.3%) had vaccination card. BCG (tuberculosis) vaccine had the greater coverage (99,3%), followed by Hb, pentavalent, and polio (>98%). All biologics showed absolute coverages greater than 90%, except for yellow fever (82.6%) and 5-year boosters (70% for MMR [measles, mumps, rubella] booster and 73% for polio and DPT booster). Age-appropriate timely vaccination coverages were lower than absolutes for all biologics. BCG and hepatitis B had coverages of 97% and Although most vaccination coverages were greater than 90%, probably the mobility restrictions due to the pandemic severely impacted the timing of vaccination. Other plausible reasons are the timely flow of vaccines (opportunity in administration and vaccine shortages).

Objective: Gestational anaemia (GA) is common in developing countries. This study assessed the relationship of late GA and negative perinatal outcomes in participants recruited in a reference maternity unit of the Caribbean region of Colombia. Design: Prospective analytical birth cohort study. Maternal Hb and serum ferritin (SF) levels were measured. GA was defined as Hb levels <6·82 mmol/l (<11 g/dl), SF depletion as SF levels <12 µg/l. Birth outcomes such as low birth weight (LBW), preterm birth (PB) and small for gestational age (SGA) were examined. Setting: Mothers in the first stage of labour, living in urban or rural areas of Bolívar, were enrolled in an obstetrical centre located in Cartagena, Colombia. Blood and stool samples were taken prior delivery. Maternal blood count, SF levels and infant anthropometric data were recorded for analysis. Participants: 1218 pregnant women aged 18-42 years and their newborns. Results: Prevalence of GA and SF depletion was 41·6 % and 41·1 %, respectively. GA was positively associated with poverty-related sociodemographic conditions. Prenatal care attendance lowered the risk of PB, LBW and SGA. Birth weight was inversely associated with Hb levels, observing a -36·8 g decrease in newborn weight per 0·62 mmol/l (or 1 g/dl) of maternal Hb. SF depletion, but not anaemia, was associated with PB. SGA outcome showed a significant association with anaemia, but not a significant relationship with SF depletion. Conclusions: Birth weight and other-related perinatal outcomes are negatively associated with Hb and SF depletion. Prenatal care attendance reduced the risk of negative birth outcomes.

We aim to describe the sociodemographic and clinical characteristics and treatment patterns of patients with Atopic Dermatitis (AD) in Colombia, due to the lack of epidemiological information about this dermatitis in our country. An observational, descriptive, and cross-sectional study was carried out. The data used was from the first phase of the National Registry of Patients with Atopic Dermatitis in Colombia (RENDAC, for its acronym in Spanish). This data was collected between July and September 2021. Absolute and relative frequencies were estimated. In addition, we estimated contingency tables using the SCORAD variable, which shows the level of severity of the AD. A total of 644 patients were analyzed, 345 were women (53.6%) and 390 were under 18 years (60.6%). Respecting social variables, we found that 81.5% live in urban areas (n=524), and 60.9% (n=392) have health care access through a privacy health insurance company. In 235 patients (36.5%) was found other skin comorbidities. The 27,5% of patients (n=177) had severe AD and are treated mainly with high potency topical corticoid and systemic treatment. After one-year, RENDAC show mainly patients of young age, female, belonging to urban areas and being affiliated to a privacy health insurance company. It is necessary to enlarge this sample to really understand the epidemiological behavior of AD in Colombia.

To estimate and analyze cancer mortality trends in Colombia, 1979–2019. Ecological study of cancer mortality trends in Colombia. We obtained individual and anonymized records from death certificates gathered by the Colombian statistics bureau. Crude and age-standardized mortality rates (AMR) were calculated expressed in deaths per 100,000 inhabitants. For the adjustment of mortality rates by age, the direct method was used. The trend in mortality rates by sex and age was modeled using Joinpoint regression analysis. Annual percent change (APC) and the estimated. Between 1979-2019, 1,194,207 of cancer deaths occurred in Colombia (annual average: 29,127 deaths), 51.6% were in women (p <0.05). For men, AMR diminished from 110.4 in the 80s to 89.7 in 2019, and from 125.7 to 79.1 per 100.000 in women, respectively. In 65+, age-specific mortality rates were higher for men than for women. In men, between 1979-1981, AMR increased 16.1% (APC: 16.1% 95%CI 8.1-24.6) per year, and between 1981-1995 increased 0.6% (APC 0.6% 95%CI 0.2-0.9) per year. From 1998 to. The downward trend in cancer mortality rates in Colombian population is probably due to variations in prevalence and changes in risk factors such as fatality associated with access to healthcare services, as well as demographic and environmental changes.
