Breathing technique in pain and cognitive function: a systematic review of the literature

Breathing techniques are key components of yoga, meditation and relaxation practices that are well known for reducing anxiety and improving overall well-being. To evaluate the efficacy of breathing techniques in pain and cognitive function. We conducted a literature review searching the main literature databases (medline, lilacs, Cochrane library) including randomized clinical trials. We assessed the risk of bias of the included studies using the methodology proposed by the Cochrane collaboration. In total, we found 16 studies that met the inclusion criteria, with an intermediate or unclear overall risk of bias. When combining the different breathing techniques vs control in the included studies, we found a statistically significant difference in terms of the visual analog scale (Difference of means, random effects; -1.21 [95% CI -1.75 to -0.68]; I2: 95%). Meditation-based breathing techniques would improve pain and cognitive function in patients with a painful entity or healthy volunteers. El presente trabajo es de carácter jurídico – reflexivo, y se aborda desde la investigación cualitativa con revisión bibliográfica. Se centra en describir y analizar un vacío legal ignorado por gran parte de la comunidad científica, pues, no lo han considerado como tal, y para efectos de este artículo, se denominará la prueba del dolor. Así mismo, se plantea como propuesta para solucionarlo, las herramientas y técnicas de la neurociencia, que permiten la cuantificación del dolor y su respectiva valoración económica. De tal manera, se proyecta la idea de una relación ineludible entre el Derecho y el desarrollo de las ciencias exactas, posibilitando la construcción de nuevos conceptos que en últimas, contribuirán al progreso del Ordenamiento Jurídico.

Objetivo. Describir la relación entre variables neuropsicológicas y niveles de cortisol sérico; como una medida de estrés fisiológico; en pacientes con fibromialgia. Metodología. Se seleccionó intencionalmente una muestra de 60 mujeres: 30 con diagnóstico de Fibromialgia y 30 controles normales. Se determinaron los niveles de cortisol mediante dos muestras de sangre (AM y PM) y se realizó una batería neuropsicológica y emocional, con un protocolo estandarizado en población colombiana para evaluar diferentes dominios cognitivos. Se hicieron análisis comparativo y correlacional no paramétrico, un análisis de regresión múltiple para determinar influencias entre las variables. Resultados. Se encontraron diferencias significativas entre los grupos de estudio en las variables neuropsicológicas (atención, memoria, lenguaje, praxis viso-constructiva y funciones ejecutivas (FE), (p<0,05), obteniendo mejores puntajes el grupo control. Se hallaron correlaciones significativas entre el perfil de cortisol, con falsos reconocimientos del test de aprendizaje auditivo verbal de Rey y con errores perseverativos del test de Wisconsin. El análisis de regresión múltiple predice la influencia de las variables de memoria y FE en el perfil de cortisol en un 88,7%. Conclusiones. Los hallazgos demuestran que, en pacientes con FM, existen alteraciones neuropsicológicas, principalmente en funcionamiento ejecutivo (flexibilidad cognitiva) y memoria episódica (evocación y almacenamiento). Igualmente, la disfunción ejecutiva está relacionada con el estrés fisiológico de manera recíproca y a su vez son condicionadas por alteraciones emocionales como síntomas de depresión, lo cual soporta el modelo neurofisiológico que compromete el eje hipotálamo-hipófisis-adrenal y la corteza pre-frontal, rica en receptores de córticoesteroides.

Objective: To identify the relationship between hemineglect and pain. Methods: We conducted a systematic review with meta-analysis, examining the relationship between hemineglect and pain. The PRISMA criteria and the Covidence platform were used to select articles and extract data. Methodological quality was evaluated, a comparison was made using pain as an outcome measure, assessed with the visual analog scale (VAS). Results: Out of a total of 340 studies, 4 were selected, 2 of which made it possible to unify pain measured through VAS; the effects were summarized using standardized mean differences (SMD), yielding -0.65, with values of [-1.02, -0.29], with statistical significance. Conclusions: There is moderate evidence of the relationship between negligence and pain. Despite the fact that from the clinical care processes it is presented regularly, and even the little scientific information available, it was established that there are specific pathologies that carry negligence and pain.

Fibromyalgia (FMS) is a chronic condition that encompasses widespread pain associated with cognitive impairment and significant emotional distress related to functional disability. This study aimed to obtain evidence of the role of pain in the effect of time since FMS diagnosis and cognitive performance using a novel online protocol of neuropsychological evaluation since the COVID-19 pandemic has challenged traditional neuropsychology testing leading to the need for novel procedures transitioning to tele-neuropsychology. A sample of 70 adult women was evaluated (50 with FMS and 19 controls) using online questionnaires that evaluated pain and executive functioning (impulsivity, inhibition control, monitoring, and planning). Afterward, participants were evaluated by trained neuropsychologists in a 30 min online session using virtually adapted cognitive tests: the Hopkins Verbal Learning Test (memory), the Symbol-Digit Modalities Test (attention and speed processing), the F-A-S test (verbal fluency), and Digit Span tests (working memory). We found that the time of FMS diagnosis has an effect on cognitive functioning predominantly mediated by pain. Our results point out the role of pain as a mediator on cognitive performance, specifically in executive functions which are directly affected by the cumulative effect of the time of diagnosis. Furthermore, the importance of considering a broader perspective for assessment and treatment including novel procedures via tele-neuropsychology.
